The apolipoprotein A1 (ApoA1) test measures the structural protein of HDL particles, so it approximates HDL particle number, while HDL cholesterol measures their cargo. Practitioners pair it with ApoB to calculate the ApoB/ApoA1 ratio, the strongest lipid predictor of heart attack in the INTERHEART study. Oral oestrogen, pregnancy and alcohol raise it and smoking lowers it, so read it with medication history.

What are the Apolipoprotein A1 (ApoA1) reference and optimal ranges?
| Typical lab reference | Vively optimal | |
|---|---|---|
| Men | About 1.1 to 2.0 g/L | Above 1.2 g/L |
| Women | About 1.2 to 2.3 g/L | Above 1.4 g/L |
Labs differ, so use the range printed on the client's report (RCPA guidance). The optimal thresholds flag a lower HDL particle count worth working on; they are not diagnostic cut-offs, and there is no benefit in chasing very high values.

Enter the value from your client's lab report to see where it sits against the typical lab range and the Vively optimal range.
A guide only, not a diagnosis. One result is a starting point: read it with symptoms, history and a repeat test. Labs differ, so check the result against the range on your client's report. Nothing you enter is stored or sent.
How do you arrange Apolipoprotein A1 (ApoA1) testing through Vively?
- Get verified. Practitioners with a recognised qualification or membership (ATMS, NHAA, ANTA, Dietitians Australia, NSA, CMA or Ahpra registration) can request free practitioner access.
- Choose a panel. Log in to the practitioner platform and pick a panel that includes Apolipoprotein A1 (ApoA1), or build your own. Add-on pricing shows when you sign up.
- Client collects. Your client collects at a partner collection centre, with their request on their phone or printed.
- Review results. Results come back to you in the practitioner platform, beside your client's other markers, ready for the follow-up consult.

Why does the optimal range sit there?
Our thresholds sit in the upper part of the reference range, where large cohorts found lower heart attack risk, particularly when ApoB was also low.
The AMORIS cohort (Walldius, Lancet 2001) and INTERHEART (McQueen, Lancet 2008) found lower myocardial infarction risk in this range. The threshold for women is higher because oestrogen raises ApoA1 production.
Drugs that raised HDL, such as niacin and CETP inhibitors, did not reduce events. The aim is to change what lowers ApoA1, mainly insulin resistance, high triglycerides, smoking and inactivity.
Vively's clinical view is that the ApoB/ApoA1 ratio holds up in people with or without heart disease.
What should Apolipoprotein A1 (ApoA1) be read with?
- LowLow ApoA1, high triglycerides, high fasting insulin
Typical insulin resistance pattern, as triglyceride-rich particles speed HDL breakdown.
- LowLow ApoA1, high ApoB (ratio above 0.7)
Plaque-building particles outnumber HDL particles; read with Lp(a), blood pressure and family history.
- HighHigh ApoA1 with high GGT or MCV
Regular alcohol intake raises ApoA1 and HDL, so a high value in this context is not reassuring.
- LowLow ApoA1 with low albumin or raised hs-CRP
Reduced liver synthesis or ongoing inflammation; needs GP review.
- ContextApoA1 normal, HDL cholesterol low
HDL particles may be small and cholesterol-poor, which is common with high triglycerides.
What can affect a Apolipoprotein A1 (ApoA1) result?
- Oral oestrogen (combined pill, oral HRT) and pregnancy raise ApoA1.
- Alcohol raises it in a dose-related way.
- Smoking lowers it; anabolic steroids lower it markedly.
- ApoA1 falls during acute illness and inflammation.
- Aerobic exercise, weight loss and fibrates raise it gradually.
When should you refer the client to their GP?
- ApoA1 below about 1.0 g/L, especially with high ApoB, LDL cholesterol or Lp(a), or a family history of early heart disease.
- Extremely low ApoA1 and HDL cholesterol, which can point to a rare inherited HDL disorder or significant liver disease.
- Low ApoA1 with low albumin, abnormal liver enzymes or symptoms of liver disease.
How should a client prepare for a Apolipoprotein A1 (ApoA1) test?
Copy or print this to send to your client.
Preparing for your Apolipoprotein A1 (ApoA1) test
- This test doesn't need fasting on its own.
- If your practitioner has requested the Full Body Health Check, fast for 8 to 12 hours before a morning collection (water is fine).
- Avoid testing while you have an acute illness, such as a cold or flu. Book for once you have recovered.
- Take your pathology request to any of 4,000+ collection centres. Bring it printed or on your phone.
- Keep taking prescribed medicines as usual unless your doctor tells you otherwise.
- Results usually take about 5 to 10 business days and come to your practitioner. Book your results appointment for after that.
How do practitioners use Apolipoprotein A1 (ApoA1) with clients?
ApoA1 responds slowly. Regular aerobic activity, weight loss where needed, not smoking, and a diet with more fibre, nuts, olive oil and oily fish and fewer refined carbohydrates lift it over months, so retest at three to six months and read it as a ratio with ApoB.
Apolipoprotein A1 (ApoA1) FAQs
Can a naturopath, nutritionist or dietitian order ApoA1 through Vively?
Yes. Add it to any client panel on the Vively practitioner platform. It sits in Lipid Studies and is also in Full Body Health Check. Add-on pricing shows when you sign up. No account yet? Request free access.
Should I order ApoA1 if I already have HDL cholesterol?
Order it when you want the ApoB/ApoA1 ratio, or when HDL cholesterol looks out of step with the client's metabolic picture. ApoA1 reflects particle count and HDL cholesterol reflects cargo, and the two diverge with high triglycerides. For a client with an unremarkable lipid panel and no risk factors, HDL cholesterol is usually enough.
How do I calculate and read the ApoB/ApoA1 ratio?
Divide ApoB by ApoA1, both in g/L. We aim for a ratio below 0.7; higher values mean atherogenic particles outnumber HDL particles. Published cut-offs vary by study and sex, so use the ratio as one input alongside Lp(a), blood pressure, smoking, glucose markers and family history.
What can push ApoA1 up and make a result look better than it is?
Oral oestrogen (the combined pill or oral HRT), pregnancy and regular alcohol all raise ApoA1. A high value alongside a high GGT or MCV is therefore not reassuring, because alcohol lifts both ApoA1 and HDL. Smoking and anabolic steroids lower it, and acute illness can drop it temporarily, so check these before reading the number.
What does a normal ApoA1 with a low HDL cholesterol tell me?
It suggests the HDL particles may be small and cholesterol-poor rather than too few, which is common when triglycerides are high. ApoA1 approximates particle number while HDL cholesterol measures the cargo, so the gap between them is informative. Read both with triglycerides and fasting insulin to see whether insulin resistance is behind it.
