Mounjaro is a prescription-only tirzepatide medicine with Australian indications for type 2 diabetes, chronic weight management and certain adults with obstructive sleep apnoea. Those indications have different clinical requirements. This guide summarises current Australian product information, dated access information and trial findings; it does not assess whether the medicine is suitable for you.

What is Mounjaro approved for in Australia?

Mounjaro has separate Australian indications for insufficiently controlled type 2 diabetes, chronic weight management, and moderate to severe obstructive sleep apnoea in adults with obesity. The diabetes indication is an adjunct to diet and exercise, alone when metformin is unsuitable or alongside other diabetes medicines. [1]

For chronic weight management, the adult indication requires an initial BMI of at least 30 kg/m², or at least 27 and below 30 with a weight-related condition. Examples include hypertension, dyslipidaemia, sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. The label also specifies a reduced-calorie diet and increased physical activity. [2]

Meeting a BMI threshold does not automatically establish that a prescription is appropriate. A prescriber considers the indication alongside medical history, other medicines and precautions. The sleep-apnoea indication is specific; it does not describe every person who snores or has disturbed sleep. The current product information is the reference for the exact scope. [1]

How does tirzepatide work?

Tirzepatide acts at GIP and GLP-1 receptors involved in glucose regulation and appetite. Its effects include glucose-dependent insulin secretion, reduced glucagon secretion and delayed stomach emptying. These mechanisms do not establish an individual outcome or make the medicine interchangeable with another product. [1]

Semaglutide has a different receptor profile and its products have their own indications and doses. Our overview of injectable medicines in Australia separates those product roles. The Ozempic guide describes that product specifically, without a recommendation to switch.

What dosing does the Australian product information describe?

The adult PI describes 2.5 mg once weekly for the first 4 weeks, followed by 5 mg once weekly. If needed, subsequent increases are in 2.5 mg steps after at least 4 weeks at the current dose. Maintenance doses are 5 mg, 10 mg or 15 mg weekly, with a maximum of 15 mg. The 2.5 mg, 7.5 mg and 12.5 mg steps are not maintenance doses. [1]

This is a description of the label, not a schedule to follow without a prescription. Dose selection, tolerability, missed doses and changes belong with the prescriber or pharmacist. The PI permits administration with or without meals. Instructions differ between devices, and a pen's concentration is not the same thing as the prescribed dose. [1]

What do the major trials show?

The major trials studied defined populations and report group averages, not a personal month-by-month result. A weight-management trial in people without diabetes and a glucose-control trial in people with diabetes answer different questions.

SURMOUNT-1: weight management without diabetes

SURMOUNT-1 enrolled adults with obesity, or overweight with a weight-related condition, and excluded diabetes. At 72 weeks, the treatment-regimen analysis reported average body-weight changes of minus 15.0%, minus 19.5% and minus 20.9% with tirzepatide 5 mg, 10 mg and 15 mg, respectively, versus minus 3.1% with placebo. Participants also received lifestyle intervention. [3]

Those results do not predict what will happen in each month of treatment. Different analyses handle discontinuation and missing data differently, which is one reason a headline percentage needs its study context. The trial was funded by the manufacturer. [3]

SURPASS-2: glucose control in type 2 diabetes

SURPASS-2 studied adults with type 2 diabetes taking metformin. At 40 weeks, average HbA1c reductions were 2.01, 2.24 and 2.30 percentage points with tirzepatide 5 mg, 10 mg and 15 mg, respectively, versus 1.86 percentage points with semaglutide 1 mg. This was an open-label trial funded by Lilly. [4]

The comparison describes those doses and that diabetes population. It does not rank every current product or dose, and it does not establish a personal prescribing choice. An HbA1c change in percentage points also differs from the percentage of body weight lost.

Is Mounjaro on the PBS, and what does it cost?

The official PBS medicine search returned no tirzepatide listing when checked in October 2026. A TGA-approved indication, a PBS listing and a clinician's decision to prescribe are separate matters. A future assessment or recommendation is not proof that subsidised dispensing has started. [5]

For published Australian private-price context, Chemist Warehouse listed a four-dose 2.5 mg KwikPen at $279.99 and a four-dose 5 mg KwikPen at $389.99, checked October 2026. These are named pharmacy examples for exact strengths and packs, not a national monthly range. Other strengths, pharmacies and associated consultation costs can differ. [6][7]

A displayed price does not establish stock or that a prescription can be supplied. The cited pages directed customers to check store availability. Do not treat private pricing as evidence of PBS eligibility or assume an overseas discount programme is available in Australia.

What is the current supply position?

No Mounjaro entry appeared in the current TGA shortage results returned during the October 2026 check. That is a dated database observation, not a guarantee of stock at a particular pharmacy. Availability also depends on presentation and strength. A pharmacist can check the prescribed pack and current supply information. [8]

What side effects and precautions matter?

Nausea, diarrhoea, vomiting and constipation are listed gastrointestinal effects. Symptoms may occur during dose escalation, but severity and persistence matter. Severe or ongoing symptoms can lead to dehydration and kidney problems; reduced food intake and malnutrition also need assessment. A fixed fluid target from an online article is not suitable for every medical situation. [1]

Serious persistent abdominal pain needs urgent medical assessment because pancreatitis is a listed risk. Signs of a severe allergic reaction, such as breathing difficulty or swelling of the face or throat, need emergency help. The leaflet also addresses severe stomach-emptying problems and diabetic eye disease. [1]

Combining tirzepatide with insulin or a sulfonylurea can increase hypoglycaemia risk. Any adjustment to those medicines and the associated monitoring plan is the prescriber's responsibility. Tell the procedural team about tirzepatide before general anaesthesia or deep sedation because delayed stomach emptying can create an aspiration risk. [1]

TGA's updated advice states that GLP-1 receptor agonists should not be used during pregnancy. For tirzepatide, oral hormonal contraceptive users are advised to discuss switching to non-oral contraception or adding a barrier method for 4 weeks after starting and for 4 weeks after each dose increase. New or worsening depression or unusual mood changes should be reported to a health professional. [9]

How is Mounjaro stored?

Storage depends on the presentation. The current PI specifies refrigeration at 2°C to 8°C before use, protection from light and no freezing. For KwikPen, the in-use unrefrigerated limit is up to 30 days at no more than 30°C. The single-dose vial and autoinjector have a different unrefrigerated limit of 21 days. [1]

Follow the leaflet for the exact pack and ask a pharmacist about temperature excursions or travel. Do not transfer storage limits, needle instructions or dose-selection techniques from another presentation. A damaged or improperly stored product needs professional advice rather than a guess based on how it looks.

What monitoring may a doctor arrange?

A clinician may use blood tests and clinical review according to the indication, other medicines and symptoms. Vively publishes information on fasting glucose testing and HbA1c testing. These tests do not independently establish suitability for a prescription or replace the prescribing clinician's monitoring plan.

The PI does not require glucose self-monitoring to adjust tirzepatide itself, although monitoring can be needed when insulin or sulfonylureas are involved. A monitoring plan should distinguish those situations. [1]

Frequently asked questions

Is Mounjaro approved only for diabetes in Australia?

No. The current Australian product information includes adult type 2 diabetes, chronic weight management within specified BMI and comorbidity criteria, and moderate to severe obstructive sleep apnoea in adults with obesity. These are separate indications. Their inclusion does not mean the medicine is suitable for every person who has one of those conditions.

Does the lowest-dose price describe the cost of ongoing treatment?

No. The quoted private price is for an exact strength and pack at a named pharmacy, checked October 2026. A different prescribed strength may have a different price. Consultation and dispensing arrangements can also vary. A starter-pack example should not be presented as a fixed monthly cost for everyone.

Can Mounjaro affect the contraceptive pill?

Yes. Current Australian warnings say reduced oral contraceptive effectiveness cannot be excluded. The advice includes discussing non-oral contraception or an additional barrier method after starting and after dose escalation. The specific timing appears above and in the leaflet. Pregnancy plans, vomiting or diarrhoea and other medicines also need discussion with your clinician.

Can I use trial averages to predict my weight loss?

No. Trial results describe a study population receiving specified doses, follow-up and additional care. They do not establish a personal monthly timeline or guarantee an outcome. Tolerability, treatment changes and other health factors also matter. The prescriber reviews response and safety; an article cannot decide when a dose should change.
