What is oestradiol (estrogen)?
Oestradiol is the most active form of oestrogen and the main female sex hormone during the reproductive years. It is produced mainly by the ovaries in premenopausal women, with smaller amounts made by the adrenal glands, fat tissue and testes in men. Blood tests measure oestradiol in picomoles per litre (pmol/L) in Australia, and the result is highly dependent on your sex, menstrual cycle phase, menopausal status and whether you are pregnant.
Oestradiol testing is commonly used to assess menstrual irregularities, fertility, menopause, unexplained symptoms such as hot flushes or low mood, and hormonal effects of certain medications. Because oestradiol shifts across the cycle and life stages, it is best interpreted alongside other hormone markers rather than as a stand-alone number. Why does oestradiol matter for long-term health and wellbeing?
Oestradiol influences far more than reproduction. It plays a central role in bone health, cardiovascular health, brain function, mood, skin, joint comfort, energy, sleep and metabolism, including insulin sensitivity and cholesterol levels. This is why perimenopause and menopause, when oestradiol drops, are associated with rising risks of osteoporosis, cardiovascular disease, weight gain, insulin resistance and mood changes.
Oestradiol also matters for men, where it supports bone health, libido, mood and cognitive function in balance with testosterone. Because oestradiol touches so many systems, tracking it in the context of symptoms, cycle patterns and other hormones can give useful preventative insight, particularly during hormonal transitions. What is an ideal oestradiol level?
There is no single ideal number, since oestradiol varies dramatically across the cycle, pregnancy, menopause and by sex. In premenopausal women, typical Australian reference ranges are roughly 100 to 400 pmol/L in the early follicular phase, rising to 1000 pmol/L or higher around ovulation, and 130 to 950 pmol/L in the luteal phase. In postmenopausal women not on hormone therapy, oestradiol is usually low, often below 100 pmol/L.
Adult men typically sit around 40 to 160 pmol/L. Interpretation depends on your age, sex, cycle phase, pregnancy status, medications (particularly hormonal contraceptives, HRT and aromatase inhibitors), symptoms and other hormone markers such as FSH, LH, progesterone and testosterone. A trend over time and in context is often more informative than a single reading.
What influences oestradiol levels?
Menstrual cycle phase is the biggest short-term influence in menstruating women, with levels shifting significantly across follicular, ovulatory and luteal phases. Life stage matters too, since oestradiol changes markedly through puberty, pregnancy, breastfeeding, perimenopause and menopause. Body composition plays a role, as fat tissue produces oestrogen through the enzyme aromatase, so higher body fat can raise oestradiol, particularly after menopause.
Nutrition, stress and exercise also influence oestradiol, with severe undernutrition, very low body fat, over-training and chronic stress capable of suppressing ovulation and lowering oestradiol. Alcohol, smoking, sleep quality and gut health all influence how oestrogens are metabolised and cleared. Medications including combined oral contraceptives, HRT, tamoxifen, aromatase inhibitors, GnRH agonists, some antidepressants and antipsychotics can all shift oestradiol, as can liver and thyroid conditions.
What are the symptoms of low oestradiol?
Low oestradiol in women can cause hot flushes, night sweats, vaginal dryness, low libido, painful sex, sleep disturbance, mood changes such as low mood or anxiety, brain fog, joint aches, palpitations, dry skin, thinning hair and increased urinary urgency. Menstrual cycles may become irregular, lighter, less frequent or stop altogether. Longer-term effects include accelerated bone loss and increased cardiovascular risk.
In men, low oestradiol (which is usually made from testosterone) can contribute to low libido, mood changes, reduced bone density and joint discomfort. Symptoms are rarely specific, so testing and clinical context are more reliable than symptom checklists.
What causes low oestradiol?
The most common cause of low oestradiol in women is perimenopause and menopause, as the ovaries produce less oestrogen. Premature ovarian insufficiency (POI), where menopause occurs before age 40, is another important cause and requires medical review. Hypothalamic amenorrhoea from severe undernutrition, extreme exercise (including relative energy deficiency in sport, or RED-S), rapid weight loss or chronic stress can also lower oestradiol by suppressing the signals that drive ovulation.
Other causes include hyperprolactinaemia (often from a pituitary microadenoma or certain medications), thyroid dysfunction, Turner syndrome, chemotherapy or radiation to the ovaries, ovarian surgery and medications such as aromatase inhibitors and GnRH agonists used for endometriosis and some cancers. In men, low oestradiol can result from low testosterone, aromatase inhibitor use or pituitary problems. Any suspected low oestradiol in a younger woman or unexplained low levels warrants medical review.
What are the symptoms of high oestradiol?
High oestradiol in women can cause heavy or prolonged periods, breast tenderness, bloating, fluid retention, headaches or migraines, mood swings, irritability, anxiety, sleep disturbance and weight gain around the hips and thighs. Existing conditions such as endometriosis, adenomyosis, uterine fibroids and PCOS may worsen, and premenstrual symptoms may become more intense.
In men, high oestradiol can cause breast tissue growth (gynaecomastia), low libido, erectile difficulties, mood changes, reduced facial and body hair, and reduced muscle mass. Symptoms overlap with many other conditions, so results always need to be interpreted alongside your clinical picture.
What causes high oestradiol?
Pregnancy is by far the most common physiological cause of very high oestradiol. Beyond pregnancy, higher-than-expected oestradiol in women can be driven by ovarian cysts (particularly functional cysts), ovarian tumours (rare), obesity (through increased conversion of androgens to oestrogen in fat tissue), liver disease (which reduces oestrogen clearance) and hormonal medications such as combined oral contraceptives, HRT and fertility treatments. Some tumours can also produce oestrogen and are rare but important causes.
In men, high oestradiol commonly reflects obesity (through aromatase in fat tissue), heavy alcohol use, liver disease, use of certain medications, testosterone therapy or, rarely, oestrogen-producing tumours. What does it mean if oestradiol is outside the optimal range?
A low oestradiol result may reflect normal perimenopause or menopause, or point to premature ovarian insufficiency, hypothalamic amenorrhoea, hyperprolactinaemia, thyroid dysfunction or medication effects. It does not diagnose any single condition on its own but is a signal worth exploring, particularly in younger women or when accompanied by significant symptoms. Long-term low oestradiol can contribute to reduced bone density and higher cardiovascular risk, so it deserves attention.
A high oestradiol may reflect pregnancy, cycle phase, ovarian cysts, obesity, liver disease, medications or, rarely, hormone-producing tumours. Because oestradiol changes so much across the cycle and life stages, timing and context are essential to interpretation. Patterns over time, alongside FSH, LH, progesterone and other markers, are far more informative than any single reading.
Can oestradiol be normal but something still be wrong?
Yes, a normal oestradiol does not always rule out hormonal or reproductive issues. In perimenopause, oestradiol can fluctuate widely from day to day, so a single "normal" result may not reflect what is happening across the whole cycle. Conditions such as PCOS, endometriosis, thyroid dysfunction and prolactin issues can affect fertility and symptoms even when oestradiol looks normal.
Timing of the test is critical, since results can only be meaningfully interpreted when the sample is taken at the right point of the cycle. Medications such as combined oral contraceptives can suppress natural oestradiol production and reduce the value of a single test. This is why oestradiol is best interpreted alongside FSH, LH, progesterone, prolactin, TSH, testosterone and, where relevant, AMH, rather than in isolation.
What other markers should be checked with oestradiol?
FSH (follicle-stimulating hormone) is an important companion, since it rises as ovarian function declines in perimenopause and menopause, and helps distinguish menopausal transition from other causes of low oestradiol. LH (luteinising hormone) supports interpretation of cycle patterns and conditions such as PCOS. Progesterone, ideally measured in the luteal phase, confirms whether ovulation is occurring.
Prolactin, TSH and free T4 are often checked alongside oestradiol, as high prolactin and thyroid dysfunction can both affect cycles and fertility. Testosterone, SHBG and free androgen index add insight, particularly in PCOS or when symptoms suggest androgen excess. AMH (anti-Mullerian hormone) reflects ovarian reserve, and metabolic markers such as HbA1c, fasting insulin, lipids and hs-CRP, along with vitamin D, ferritin and B12, complete the wider picture. How can you improve oestradiol to a healthier level?
Because oestradiol is a hormone produced and regulated by complex feedback loops, it cannot always be self-optimised, and any hormonal treatment (such as HRT or the combined oral contraceptive) should always be prescribed and monitored by your GP or specialist. That said, supporting overall hormonal health through balanced nutrition (with adequate protein, healthy fats, fibre, phytoestrogen-containing foods such as legumes and flaxseed, and micronutrients like iron, iodine, zinc and vitamin D), regular movement and appropriate rest is helpful. When does oestradiol need medical review?
See your GP if you have persistent hot flushes, night sweats, irregular or absent periods, vaginal dryness, low libido, unexplained fatigue, mood changes or brain fog, particularly if you are under 40 (which may suggest premature ovarian insufficiency). Any menopause symptoms significantly affecting quality of life, difficulty conceiving, recurrent miscarriages or symptoms suggestive of PCOS, endometriosis, thyroid dysfunction or high prolactin also warrant clinical review.
Very heavy or prolonged periods, breast changes, gynaecomastia in men, unexplained galactorrhoea (nipple discharge unrelated to breastfeeding), pelvic pain or symptoms of ovarian cysts or tumours need prompt medical assessment.
